Actually, I saw on one of the New Year's specials last year that one of the new trends in Shibuya was a 男子高生 cafe (a cafe with high-school aged boys) which basically served the same function for the ladies. The premise was that all of the boys there go to the same (fictional) high school and all work there to make money so they can get up the courage to ask out the girl they like.

I found it to be rather ... bizarre. YMMV.

From what I understand, it isn't terribly common over there, though I've heard some anctedotal claims that it just doesn't get reported.

I could name a couple reasons that might be off the top of my head but it'd be just speculation at my part, but from what I understand, the girl's are actually pretty safe.

@everybest: In Japan, cleaning each other's ears is supposedly to be an intimate thing for couples, since it's so personal. Seems to me they offer that to draw in the guys who are just lonely and want to hang out with a lady for a while.

It's kind of like a PG-13 strip club, only without the nudity or the booze.
