Is that Wonder Boy in Monster World for the Mega Drive? I don’t remember a fairy following you around, but the character looks right.
And even if I’m wrong, didn’t you end up fighting aliens at the end of that game?

Yeah, you picked up the fairy from the queen through the gate in the town pictured. She bashed things for you and found hearts along the way.
The aliens at the end were really crazy. Took me ages to get past the final boss and none of my friends were able to do it. I think I was lucky as I couldn’t do it when I retried.
The ending was really crap to, but the game was brilliant, so I could forgive it.
Wonderboy in Monster World for the MegaDrive of course! I have all the Wonderboy games (except the japanese only Monster World 4) on cartridge for SMS/MD, with my personal favourite being the classic, Wonderboy 3: The Dragons Trap on SMS.

Had a megadrive for about 6 months back in the day, cracked this and Ecco
How sweet would a HD Ecco be……-
A HD Ecco would be fantastic, as long as it’s kept 2D. They tried 3D on the Dreamcast & PS2, and while it looked stunning, it was incredibly difficult and very hard to control.

Yeah! WonderBoy in Monster World is one of my favourite games of all times but (fun fact)as I grew up in Brazil I know this game as Monica na terra dos monstros (Monica in Monster World).
Monica is the most famous comic book in Brazil, and only last year that I was able to finish the real version of this game
Wonderboy 3: The Dragon’s Trap was one of my favourite games growing up.
